HeartLand Family Wellness FSP Referral Process

Appropriate referrals include families with negative and highly complex family dynamics frequently characterized by housing insecurity, psychiatric hospitalizations, co-occurring substance use disorder/s, incarceration and involvement with the criminal justice system, intergenerational trauma, high ACEs scores, and other risks and vulnerabilities associated with poverty. Referral Criteria – Required

♡ At least one person in the family must meet target population and medical necessity criteria and consent to receive services.

♡ At least one youth under 21 years of age must be living in the home or will imminently be living in the home.

♡ Caregiver and/or youth identify a need for Family FSP services and consent to the frequency/intensity of services.

 

Referral Criteria – Identified Risk Factors

Struggling families come in many shapes and sizes and often face an array of challenges, especially as it relates to poverty and the inability to fully utilize available resources. The risk factors as outlined below are not meant to be all inclusive, but to provide guidance as to the intensity of challenges appropriate for referral and the level of intervention offered by the Family Wellness FSP.

 

Referral Criteria – Examples of Risk Factors

♡ Clear compromise of ability to care adequately for oneself, despite prompting

♡ Child Welfare Involvement

♡ History of high-risk behavior / impulsivity within last 6 months

♡ Fleeing violence (domestic violence, war, genocide)

♡ Structural and Institutional Racism

♡ Suicidal / Homicidal Ideation: Recent or current, inactive or active ideation, intent, or plan within the past 12 months

♡ The family experiences intergenerational trauma that may include high ACES scores, factors found in the school to prison pipeline, eating disorders and/or court-ordered mental health treatment

♡ The youth and/or caregiver is at risk for loss of educational / vocational placement due to emotional and behavioral challenges

♡ Experiencing vulnerabilities and risks associated with poverty that have a destabilizing effect thereby reducing ability to obtain financial assistance, including housing

♡ Experiencing imminent threat of homelessness

♡ Sleeping in a place not meant for human habitation or an emergency shelter (e.g., car, van, camper not hooked up to facilities, encampments, storage shed, etc.)

♡ Hotel/motel paid for by non-profit/county funding
